24Care Australia is a disability support and services provider in ACT, VICTORIA, QLD AND NSW. We are a value based and service-oriented organization. 24Care Australia is an organization that was established with the sole aim of offering disability support and Services 7 days a week, 24 hours a day that puts the participant as a priority in promoting independence and progression in a safe and non-discriminative environment. At 24Care Australia we provide a range of disability services. With the range of services we provide, we aim to make a difference in the lives of those with disability. The goal is to make each of our participant’s lives easier, fairer and more involved in their own communities. Home Care Our Support Worker and care staff are experienced and trained to handle full range of services on a live-in or live-out basis, temporarily or long term to suit your needs and circumstances. Our service include but not limited to; Assistance with daily personal activities Assistance with travel/transport arrangements Community nursing care for high care needs Development of daily living and life skills (Co-Ordination of supports/Case management) Household tasks Co- Ordination of Supports: Help with connecting with providers of supports so that your goals and expectations are met. Depending on your needs and budget, we can be as hands on as you require.
