In addition, we launched our Transitional Care Program™ to Healthcare Providers in 2015, designed specifically for transitioning all levels of complex patients from an acute setting to a community that meets their specific care requirements. Our Program has proven to be an effective and highly successful solution to meet the needs of both the patients and healthcare providers. Our mission is to be a leader in providing innovative solutions and ethical practices to support the transition of care for any patient. Our models reduce length of stay and significantly reduce re-admission rates. We plan and coordinate all services needed for the most optimal and effective outcomes for our clients and partners. We take the time to build strong relationships with our partners and locate proper resources in the community to ensure we are recommending the most appropriate options to our clients.
