ABBOTTCARE: VISION & MISSION AbbottCare is Abbott Home Health Care, a CHAP accredited, Medicare certified visiting nurse agency, and Abbott Senior Care, a private, supportive care agency that provides services to assist clients to live safely at home. We are also sub contractors to HESSCO Elder Services, Old Colony Elder Services, South Shore Elder Services, the Mass Rehab Commission, MassHealth, and the VA home care programs, providing home health aides, homemakers and companions. In 2011 and 2015, we were chosen for Medicare’s HomeCare Elite as one of the top home health care agencies in the U.S. Relationships are the heart of patient centered care. At Abbott, we have a philosophy of ONE nurse, ONE patient. It improves continuity of care and improves outcomes. Our registered nurses serve as our case managers getting to know the patient and family, allowing for one point of contact for the patient and the primary care physician, and fewer strangers in patient's homes. We are able to provide supportive care for as little as 1 hour or as much as 24 hours a day, 7 days a week; cover a large geographic area of Southeastern MA and answer our phone with a HUMAN BEING 24 hours a day, 7 days a week. Our team goes the extra mile to provide an exceptional level of care and service. From companion care, to personal care attendant or home health aides, and with the coordination and supervision of our skilled care staff, we are able to improve the continuity and coordination of care, with improved outcomes and better communication for the patient, family, primary care provider and caregivers.
