OUR VISION is people with a disability are empowered to live a dignified and independent life of their choosing, connected to family, friends and community. OUR PURPOSE is to provide support services to people with a disability that affirm who they are, build their capacity and create opportunity. AFFIRM CARE's SERVICES are individualised and culturally secure. We support people in the Perth metro area who have access to their own funding through the National Disability Insurance Scheme (NDIS).
