Aging Care, LLC assists families with the coordination of care for their parents or loved ones to navigate them safely through the healthcare maze. Services include: •Assessments of a parent’s physical health, functional status, medical needs, social network, cognitive function and financial issues. •Care Plan Development to develop a customized recommended care plan based on individual and families’ needs including identifying community resources and coordinating implementation •Ongoing Care Plan Management to coordinate care from community resources, coordinate with health care professionals and monitor the plan to address changing needs. •Patient advocacy to protect, promote health, ensure quality of life, and uphold the individual’s rights with health care providers, facilities and other service providers •Crisis Intervention to assess situations, identify issues and suggest appropriate resolution. •Relocation Services to help older individuals and families faced with the decision to move or not to move.
