Battersea Primary Care Network (PCN) is a vibrant and innovative healthcare organisation dedicated to transforming primary care in North Battersea. We are a collaborative partnership of five GP practices, united by a shared passion for providing exceptional care to our community of over patients. Our approach is rooted in collaboration, innovation, and a deep commitment to health equity. We believe in empowering our patients to take charge of their well-being and working together to build a healthier Battersea. Our dedicated team includes a diverse range of healthcare professionals working together to provide comprehensive care- Care Coordinators - Community Health and Wellbeing Workers - First Contact Physiotherapists - Health and Wellbeing Coaches - Mental Health Professionals - Paramedics - Pharmacists and Pharmacy Technicians - Physician Associates - Social Prescribers Our Values in Action: 1) Equity: We champion health equity for all in Battersea, fostering a welcoming and inclusive environment. Our Community Health and Wellbeing Worker (Brazil Model) project is a prime example of this commitment, empowering local residents to become health advocates within their communities. 2) Collaboration: Our PCN team works collaboratively with GPs, practice nurses, and administrative staff at each practice, as well as with community partners like Enable, Community Empowerment Network, St. George's NHS Trust, and South West London and St. George's Mental Health Trust. 3) Innovation: We embrace new ideas and technologies, pioneering models of care that address the unique needs of our diverse community. Our Children and Young Person Clinic is a testament to this, offering a unique approach to supporting young people's mental, emotional, and social well-being.
