We utilise a systems thinking approach to engineering failure, together with data analytics and engagement with our wider network of trusted partners, to deliver forensically sound expert opinion on matters of both causation and compliance.| POLICY ADVISORY | We help decision-makers in government take a systems thinking approach to addressing long-standing industry-wide challenges that incur significant community impact and cost. For example, Sean Brady was commissioned by the Queensland Government to carry out a review into all fatal accidents in Queensland mines and quarries from to 2019. The review delivered eleven recommendations and was tabled in Queensland Parliament in February 2020.
