Care Bridge Specialists believe in homes not hospitals & their vision is rooted in a fundamental belief that autistic people with associated conditions & complexities deserve access to high quality care that is strengths based, aspirational & consistent. They are a niche provider of wraparound teams that practise autism informed care in order to accelerate hospital discharges & avoid hospital admissions. They utilise whole systems thinking & partnership working with Clinical Commissioners, Complex Case Managers, HOPE(S) Practitioners, Senior Intervenors, Advocacy Services, Care Co-ordinators & MDT Teams in order to promote positive risk taking & achieve positive outcomes. Care Bridge have the expertise to provide and co-ordinate highly skilled wraparound teams to an incumbent provider when a package of care is at risk of breaking down & they offer the consistency of a bespoke team to go into hospitals to build high trust therapeutic relationships & form part of recommendations & work around discharge planning. Together with an in house IST that offers multi disciplinary clinical expertise, our frontline teams foster confidence for providers and hospitals by attending every ward round, MDT, CTR, escalation panel & 12-step discharge planning meeting. They build relational security with a team that offers consistency & continuity. They write & reviewing robust care plans & dynamic risk assessments & they use an ABC approach to ensure that PBS Plans reflect real time insights & remain least restrictive & effective. Care Bridge Specialists & their sister/ long term care arm 'Care Bridge LIFE' cover nationally with teams that travel in & out of area to make this specialist model of care work. Their work and positive impact has been recognised by Senior Intervenors in NHS England and they have been invited as one of only two specialist providers to be part of the Complex Care Roundtable panel working with NHSE for ‘Building the Right Support for Autistic People’.
