CareBridge's virtual-first program provides members and their caregivers with a cellular-enabled tablet that gives them access 24 hours a day, 7 days a week to CareBridge's interdisciplinary medical team of physicians, psychiatrists, pharmacists, nurses, and social workers. Decision Support is an innovative solution that provides actionable recommendations on the type of home and community-based services that will help members achieve optimal independence in their lives. To develop these recommendations, CareBridge's LTSS Specialists, each of whom have a background in physical or occupational therapy, complete a person-centered assessment of each individual, informed by insights from the nation’s largest HCBS analytical database, to identify potential enhancements to the individual’s care plan and recommend empowering alternatives that can keep the member safe and independent at home. CareBridge's solutions have been shown to increase independence, improve clinical quality, and reduce emergency room visits, hospitalizations and total costs all while receiving a Net Promoter Score of 84 from patients and families.
