Caring Hands Some Care Services is a boutique home care service that provides private duty nurses and home healthcare to those who have a need and would benefit from such services. We offer a full range of in-home care services to meet the needs of clients upon discharge from the hospital, rehabilitation facility, out-patient surgery center or upon recommendation of the physician. Upon completing a thorough assessment, a CHHC RN creates a personalized plan of care coordinating closely with the client and/or their representatives, family members, and the physicians. We ensure all parties are included in the Plan of Care in order to deliver the best possible care and successful outcomes. CHHC nurses and caregivers serve as the patient's advocate. CHHC has specialized programs including IVIG therapy, Infusions, TPN, tube feedings, Alzheimer's, dementia & Parkinson care as well as substance abuse recovery to name a few. We also offer our "Boutique Style' of services to handle everything from meeting your nurse before your procedure, transportation to your appointments, follow up care in your home and management of overall care. CHHC offers clients and their families the services and support needed so loved ones can remain as independent as possible and remain at home.
