Carolina Collaborative Community Care

Carolina Collaborative Community Care (4C) is a non-profit organization providing care management services to Carolina Access II and Medicaid patients in Cumberland County. We are located in historic downtown Fayetteville, North Carolina, in the Systel building. Our staff is comprised of both clinical and non-clinical positions. As an award-winning care management model, our company assists patients with medication reconciliations, coordination of services and disease state education. Our team supports patients and their physician’s offices via telephone interaction and home visits. The 4C team consists of nurses, social workers, physicians, pharmacists, pharmacy technicians and many other qualified professionals.
