CHW Cares is a neighborhood-focused company that delivers end-to end healthcare in clinics, at home and on the go. We focus on serving neighborhoods with significant populations of minority and immigrant older adults with teams of community-based health coaches who are supported by clinicians to create an excellent patient experience, deliver outstanding results, and manage medical costs. We reinvest in our neighborhoods by creating high quality frontline jobs and improving the health and digital literacy of our patients and their families. CHW Cares was founded in 2013 as City Health Works to demonstrate the power of community-based teams in delivering high quality health outcomes, significant reductions in total cost of care, and exceptional patient satisfaction (average NPS score of 90). In 2021, CHW Cares launched its first comprehensive neighborhood hub in New York City by building upon a multi-specialty practice that serves patients annually. CHW Cares will continue to scale its footprint to acquisition and modernization of existing independent physician practices, while attracting greater organic growth in the neighborhoods we serve through delivering a superior patient experience. Motivated by our vision to deliver high quality care to seniors, CHW Cares partners with insurers on value-based contracts to enable it to focus on its mission to deliver proactive home, clinic, and remote based care to seniors.
