With decades of experience in disability and mental health, Collective Support Services is committed to providing outstanding Support Coordination and Recovery Coaching services to plan and self-managed NDIS clients. We strive for personalised service, with a focus on individual needs and goals and creating a plan to meet those goals. We follow our core values of purpose, collaboration and growth in our mission to communicate clearly and effectively with clients. With a passion for person-centred services, we can guide and mentor provision of active support. Incorporating a trauma-informed care approach we are able to provide resources and guidance to develop person-centred plans, active-support plans, communication and positive behaviour strategies, all of which can result in outcome-focused progress reports.
