Community Care Plan (CCP),'the health plan with a heart,' was introduced in as South Florida Community Care Network (SFCCN), the first safety-net hospital-owned Provider Service Network. CCP serves over lives enrolled in Medicaid, Florida Healthy Kids, uninsured programs, and self-insured employee health plans. CCP is owned by Memorial Healthcare System and Broward Health. Community Care Plan maintains accreditation for its Medicaid and Florida Healthy Kids Health Plans by the National Committee for Quality Assurance (NCQA). CCP is also fully accredited by the Accreditation Association for Ambulatory Health Care as a Health Plan. Certified as a Great Place to Work In 2025, CCP announced the launch of 22 Health, a new division offering new health insurance marketplace (ACA marketplace) plans to the residents of Broward County for 2026.
