Informed by contemporary evidence, these conversations empower individuals to take action that improves mental health and wellbeing. Connect 5 is now delivered by Solent Mind, Thameside Oldham and Glossop Mind, Medway Council, Havent and East Hants Mind, Healthy Cornwall, Swindon Borough Council, Action Together, Stockport and District Mind, Cumbria Learning and Improvement Collective and Greater Manchester commissioned by Manchester Health Health and Care Partnership.
