Connectus is a non-profit community healthcare organization founded on the values of Compassion, Wellness, Community and Culturally Sensitive Care. Our staff and facilities to help manage the healthcare of the entire family and community, including individuals who are underinsured or lack insurance. At Connectus Health, we are dedicated to providing affordable, high quality healthcare that is designed around you and your healthcare needs. Connectus utilizes the Patient-Centered Medical Home (PCMH) model of care. A Patient-Centered Medical Home allows you to personalize your care plans, supports better understanding and monitoring of prescription medications, provides tailored healthcare coaching and advice, and creates stronger relationships with your healthcare team and other community resources.
