Dementia Directions

Dementia Directions supports individuals experiencing memory loss and post-dementia diagnosis for people living with dementia, carers, family members, and guardians. Services offered are tailored to the unique needs of each individual and their carer and are provided by an experienced team in dementia care, advocacy, and aged care. Our advocacy approach aims to ensure that the rights and choices of individuals with dementia are upheld and respected throughout the care journey. Services: Diagnosis Support: Case management, starting from the identification of symptoms to the diagnosis of dementia and addressing objections or barriers to seeking medical help. Advisory: Post-diagnosis in-home dementia advisory services, providing guidance and support for the person living with dementia and their carer. Aged Care Navigation: Aged care navigation to access available support privately or through My Aged Care, assisting with the complex process of accessing appropriate aged care services. Help At Home: Placement with a matched home care provider to support individuals to stay at home for as long as possible. Future Planning: An advisory service including assistance with Enduring Power of Attorney or Advanced Care Directive to ensure that the individual's wishes and preferences are documented and respected. Advocacy: Supporting individuals with dementia in having their voices heard concerning their health and well-being, rights, and choices. Dementia Directions aims to provide comprehensive and person-centred support to individuals with dementia and their carers, encompassing various aspects of care, navigation, planning, and advocacy to improve their overall well-being and quality of life.
