Elevated Supports, LLC is a Colorado-based provider agency delivering person-centered services to individuals with intellectual and/or developmental disabilities through Medicaid-funded programs. We support people to live with greater independence, dignity, and connection in their communities. Our services are designed to meet individuals where they are, while building skills, stability, and long-term success. Elevated Supports currently provides services through the following core programs: • Residential Services – Supporting individuals in host home and supported living environments with a focus on safety, autonomy, and quality of life. • Supported Living Services (SLS) – Flexible, individualized supports that promote independence in daily living, community participation, and self-direction. • Home Care Agency (HCA) – Licensed personal care services delivered under the HCBS-SLS waiver, supporting health, safety, and activities of daily living. • Supported Employment – Assisting individuals to explore, obtain, and maintain meaningful employment in their communities. • Supported Community Connections (SCC) – A robust, activity-based program offering inclusive social, recreational, educational, and wellness opportunities that foster connection and community belonging. • Benefits Counseling – Medicaid-billable benefits planning services to help individuals understand how employment and income impact public benefits and long-term financial stability. Elevated Supports works in close coordination with case management agencies, healthcare providers, families, and community partners to ensure services are compliant, responsive, and person-centered.
