Expert Pharmacist Review specialises in Home Medicines Reviews (HMRs) to improve medication management. Our accredited pharmacists have many years of experience in clinical pharmacy. We can help clients understand their medicines, provide up-to-date medicines lists, and working with other healthcare members to improve health and wellbeing of the clients. Home Medicines Reviews are fully funded by the Australian Government and are of no costs to eligible clients. We are currently performing Home Medicines Reviews in the inner suburbs of Melbourne, but can also visit people in other areas.
