Extended Managed Long Term Care

EXTENDED MLTC is committed to maintaining the highest ethical and moral standards. Our employees and providers focus on providing compassionate care while emphasizing the physical, emotional, and social needs of our members. We are able to accomplish this by partnering with quality health care professionals to promote and maintain optimal health and independence within the home and community. We are also dedicated to providing our members with information on all available treatment options while ensuring any and all translational needs are met. EXTENDED MLTC will become the preferred, community-based program dedicated to working in collaboration with members, caregivers, and providers to promote and maintain an optimal level of health and independence, allowing our members to remain in their own homes safely and with dignity. We are dedicated to achieving the highest level of teamwork amongst all parties involved to ensure that all of our members' needs are met regardless of sex, race, health status, color, age, national origin, sexual orientation, or religion. Our mission is made possible by providing home care, social services, personal care, Adult and Social Day programs, as well as health promotions. Services are offered in all five boroughs of New York City, as well as Nassau and Suffolk Counties.
