We provide personalised, community-driven services that strengthen independence, build connection, and support people across all life stages. “This year, FOCUS Connect supported more than 2,169 people across aged care and community services.” Aged Care Services As a registered Aged Care Provider, we offer multicultural support through the Commonwealth Home Support Programme and the Support at Home Program (formerly Home Care Packages) across South West Sydney and Northern Sydney. Our team provides tailored care that helps seniors live independently and with dignity. Services include: Housekeeping & Meal Preparation Lawn Mowing & Gardening Personal Care Registered Nurse Clinical Care Nursing & Medication Management Social & Community Support Multicultural Support Groups Respite for Carers Home Maintenance & Modifications Goods, Aids & Equipment 🧩 Community Support Services Our holistic, culturally sensitive programs are designed to uplift and empower individuals and families from all walks of life, removing barriers, building relationships, and fostering long-term wellbeing.
