Grace Group Home LLC is a residential home offering CL placement for clients interested in an assisted living program. At Grace, we are led by a team of proficient and compassionate caregivers, who focus on the daily needs of those residing in the home and strive to provide person-centered services to individuals in a setting full of support and comfort. We are devoted to giving individuals the freedom of choice in their daily lives and committed to building a team that is focused on and strives for success by providing quality care and changing their lives. Below is an overview of the services we offer: Services- 24-hour Customized Living in our Residential Homes- Grace Group Home enables individuals to make decisions and to learn and grow in an inviting, inclusive setting that promotes personal well-being, health, community participation and relationship-building. Services Include- Up to 24 hours of supervision - awake overnight - Activities of daily living (ADL) assistance - Assistance with mental health cognitive or behavioral concerns - Heath related assistance - Home management task - Non-medical transportation - Socialization Respite Services- We provide services to persons that are unable to care for themselves on a short-term basis in our homes.
