Healthy Care Services creates and facilitates sustainable solutions to deliver better health outcomes for your clients. Healthy Care Services (HCS) is the contract management arm of The Healthy Collective, a collaboration of health practitioners and social care providers with a vision to improve the lives of citizens and the sustainability of services in communities. HCS will do this by providing governance for service delivery enabled by agreed practice standards, shared platforms for care provision (including digital), mobile clinical teams and a central unit for contract management and practitioner support. The Healthy Collective supports health professionals to improve their practices and facilitates inter-disciplinary synergies under a single point of governance to optimise patient outcomes and value in primary care, aged care, disability and more. Launched in 2019, HCS draws on over 15 years of successful contract management and health service delivery experience to design, implement and manage the provision of multidisciplinary healthcare services for your organisation and clients. We are not a direct provider of health services, rather we broker an arrangement with aligned health practitioners that are located near to your clients and facilitate their provision of an agreed service level. Whether you are a single or multi-site operator we take the risk out of your contracting by introducing an independent level of governance for performance management; and optimise choice in provider(s) from our broad practitioner network and ability to engage independently with all parties.
