Healthy Hyde

We are Healthy Hyde who work with the 8 GP practices in Hyde. We are a Primary Care Network (PCN) supporting patients in Tameside. We have a team of wellbeing practitioners, nurses, paramedics, mental health nurses, first contact physio’s and care co-ordinators who work with your community in multiple ways. Our vision is to ensure the best possible healthcare & access to local services for all of our patients. We endeavour to make the best use of our resources to ensure that every one of our patients gets the help and support that they need and deserve.
