We specialize in high-thrive aging! Mobile outpatient rehabilitation clinic delivers evidence-based therapeutic interventions to support, rebuild and maintain whole-person wellness, independence and greater life satisfaction. We partner with physicians, senior living communities, assisted living facilities and care providers. We make house calls! We are pleased to offer the full benefit of rehabilitation therapy via our mobile outpatient therapy service and telehealth in the comfort & safety of your home. If you live in your own home, with friends or family, in a senior community or assisted living, you qualify for our mobile outpatient therapy services. PROACTIVE INTERVENTION We focus on proactive intervention that interrupts functional decline and deconditioning before serious health problems can cause long-term harm, hospitalization and loss of independence. In this era of life-saving medical advances, it is wise to consider that an older person might be medically well, yet functionally sick. We catch issues that others miss and work with the client's doctor on a plan of care that restores that person to their highest potential. OUR PHILOSOPHY We believe in the beauty and wonder of the aging process and ascribe to a paradigm shift in the approach to the care and upkeep of the body, mind and spirit of older persons. It is our earnest belief that the aging process does not have to be a trial of pain and decline, but that through early therapeutic interventions, rehabilitation and nutritional support, older people can be supported and empowered to thrive in this season of their lives.
