From direct patient care for the chronically ill, to transitional care programs from hospital to home. Home Physicians Group medical providers visit nursing homes, assisted living facilities, provide in-home appointments for home bound patients and provide hospice care. In addition, our medical providers will order lab tests, xrays, EKG’s, and whatever “ancillary” services or procedures that may be required to fully evaluate any medical problem — and you almost never have to leave your home for these to be done! Furthermore, we also work with several home health care agencies and can help provide medical supplies, physical therapy, nursing, wound care and hospice care at home.
