Hope Primary Care

Welcome to Hope Primary Care, a practice dedicated to provide high quality health care services addressing a large majority of personal health care needs, developing a sustained partnership with patients in the community. Some of the services we offer include annual wellness & physical exams, chronic (long-term) disease management – such as management of high blood pressure, diabetes, cholesterol, thyroid – common acute illnesses, women’s health, mental health concerns, sleep issues, skin conditions, and more. We focus on preventative care to help create and maintain a healthy lifestyle. When medical condition requires a specialist, we coordinate care with other, appropriate medical specialists. We also work together with Hope Dentistry and are dedicated to raise awareness about the relationship between oral health and whole body health and help improve oral and general health in our patients and communities. We promote effective provider-patient communication and encourage the role of the patient as a partner in their own health care decisions.
