Sinai School of Medicine, Institute for Care at the End of Life (ICEOL) at Duke University, and Nurses improving Care for Healthsystem Elders (NICHE) at NYU. In addition, HPDP works with Federally Qualified Health Clinics within the State of Vermont to implement processes for improved quality of care.
