Established in 2017, Impact Health PC is a physician owned, independent nurse practitioner group. With dedication to quality outcomes, clear communication, and innovation, our practitioners are involved across the continuum of post-acute care. Our consultative model provides care to patients within skilled nursing facilities and beyond through our telemedicine division. Staff consult with patients onsite for medical and psychiatric care during a residential facility admission and continue patient care after discharge to home with tele-medicine and remote patient monitoring services. Impact Health PC’s combined pre and post discharge focus avoids unnecessary hospital admissions to improve key quality metrics while enhancing patient and family satisfaction.
