Jessica Care is a privately-held, concierge care management company based in Lakewood Ranch, Florida providing care management services tailored to meet the client’s specific healthcare needs. The Company is committed to raising the standard in senior care. We work closely with families, whether living at a distance or nearby, to create and implement a care plan, specific to the needs of their aging parents. Jessica Care works closely with well-vetted physicians, nurses, home health care companies, and many other talented area healthcare professionals. Jessica Care offers expertise to clients and provides clarity during times of uncertainty. Having expertise in the senior/aging field, we understand the frustrations that families endure. We believe in a client-centered holistic approach when caring for your aging parent, spouse, or loved one. We serve as an advocate and consultant to individuals, families and those in need, based on the Guiding Principles of Social Workers: Person-centered services. The social work case manager engages the client (and, when appropriate, other members of the family system) in all aspects of case management and tailors the services to the client’s needs, preferences, and goals. Person-in-environment framework. The social work case manager understands that each individual experiences a mutually influential relationship with her or his physical and social environment and cannot be understood outside of that context. Collaborative teamwork. The social work case manager does not work in isolation.
