LIFE CHANGES ELDER CARE CONSULTING, LLC IS AN ELDER CARE MANAGEMENT AGENCY. Providing professional care management and power of attorney for health care services. We work with individuals and/or familes around the country to assess and manage specific needs being faced with life’s inevitable changes as we age. Whether you are an adult with family caregiver support, a care partner needing guidance for your aging family member, or an independent solo ager; our team is experienced in assessing, planning and taking the necessary actions to ensure quality of life needs are understood and have a plan in place to be implemented. Our initial assessment will help determine: • Issues relevant to individual’s health and living situation • Ability to perform Activities of Daily Living (ADLs) • What in-home or other care services may be needed • If residential placement should be planned for Once we have reviewed the assessment and our recommendations with everyone, Life Changes will create a detailed Care Plan with prioritized action steps and recommendations for support individuals, service providers and resources.
