Master•Care is a provider of Enhanced Care Management designed to improve clinical and financial outcomes of California's Medi-Cal population, including older adults, high healthcare utilizers, and individuals affected by homelessness. Master•Care blends expertise with non-medical senior care with healthcare and delivers services under managed care contracts. By helping managed care plans reallocate spending away from frequent E.R. visits, skilled nursing, and hospital stays towards non-medical care like personal care, home modification, and medication management, patients benefit, and medical expenditures are decreased. Master•Care's brand of Enhanced Care Management is called "Care Navigation" and is guided by each patient's "Master•Care Plan."
