We are focused on the total patient and communicate regularly with the people who matter most to you, ensuring you receive complete, quality care. We answer our phones and are responsive to your needs. Located and headquartered in Jacksonville, Florida, we are accessible, provide a community focus of holistic care, and are always here for you. We are a full service in-home primary care practice of seasoned nurse practitioners, and can also set up the following services to come to you as required:*Lab draws, routine and as needed *INR management for Coumadin *X-rays, ultrasounds, EKGs, ECHOs, sleep studies and oxygen testing *Home Health Therapy (physical, occupational, speech)*Getting much needed medical equipment (walkers, wheel chairs, hospital beds, oxygen, etc.)*Home Health Nursing (frequent health monitoring needs) Our goal is to keep you healthy and happy in your home through preventative health visits and sick visits, and whatever is required to meet your healthcare needs.
