myDMR care management software is an in-practice tool that supports value-based primary care. It complements certified EMRs to streamline workflow, address CMS requirements, and provide better outcomes for chronically-ill patients. Additionally, myDMR empowers patients to grant* healthcare providers remote access to clinical information derived from their primary care record. Patients receive a wallet card containing a QR code that allows access (ie: ED use case) to a summary care record (CCD).
