We help health systems turn existing documentation and fragmented operational work into coordinated execution, measurable outcomes, and high-quality program data, with built in AI-powered patient outreach, scheduling, and follow-up at scale. Our platform combines a unified data model, an activity layer for program operations and documentation, a node-based automation studio for triggers, alerts, forms, messaging and calls, and an intelligence layer that extracts structured data from EHR notes, scanned PDFs, external documents, and images.
