We are the resource that families call when things don’t go as planned. We assist older adults and families in understanding, accessing and coordinating available resources, care and benefits. NextPath works with families and individuals to identify care goals, develop a customized, sustainable plan of action, and see to it that the plan is followed by providers. We offer a range of services including consultation, future planning, education, guidance, benefit assistance, housing evaluation and recommendations, ongoing monitoring, family support, and expert care coordination.
