We are exclusively located in the beautiful city of Fremonty, CA, approximately 40 miles southeast of San Francisco and 18 miles northwest of San Jose. Niles Canyon Post Acute provides compassionate and personal skilled care 24 hours a day for both short and long-term residents. We specialize in providing rehabilitation services (physical, occupational, and speech therapy) onsite to help our residents regain their independence and return to their homes. We are also able to provide IV antibiotic treatment, wound care,Niles Canyon Post Acute is proud to be serving the community of Fremont and surrounding area since we have been established in 2019, and we strive daily to be the premier Skilled Nursing Facility of the Tri-City region.
