Pointcare Assessments

POINTcare Assessments provides comprehensive evaluations for individuals; families and caregivers; retirement communities; and vendors of products and services for Seniors. Our goal is assist elderly patients and their caregivers to maintain the highest level of functioning at the current point in their life. Through our POINTcare Assessments, we establish a pathway to achieve this level in the appropriate setting and with the appropriate care. Geriatric Care Management is provided as a service of our partners at Concierge at home, a single point of contact for management and coordination of the healthcare needs of our clients.
