Polycare Project

Polycare is an European project, within the Horizon 2020 research and innovation programme, that has been conceived as a new integrated care model supported by ICT for contributing to redefine and relieve the assistance model. It is been carried out by an consortium of companies that can be check at It is an initiative addressed to elderly people which most remarkable foundations/objectives of the project are to provide an integrated care while empowering the patients through collaboration among stakeholders, allowing home-hospitalization with a continuous non-intrusive monitorization of the patience condition. It did implied the need to adapt technical solutions to these end-users so that they are well accepted and help to improve their quality of life. At the same time, the new care methodology will make stronger the ties with the elderly and their formal and informal caregivers. Polycare also puts on the table a new business models for SMEs and large companies focused on deploying the technical components across Europe, and it could also help private health and social care companies to operate in a more efficient way and providing more quality in its attention. As the project is been evaluated in three pilot sites on three different countries, which will provide very valuable data as the differences among these sites are remarkable and both organizational and technological there are many KPIs that will show its progress
