Post-Acute Care Resources is a consulting service that assists providers best position their post-acute services including inpatient rehabilitation, skilled nursing, home health, outpatient rehabilitation and hospice. Our approach is to be a resource that offers extensive knowledge and years of experience in post-acute care to our clients to identify the opportunities for improvement and then, in collaboration with our clients, develop action plans to remedy the issues. Our team is prepared to assist in whatever means our clients select, whether it be validation of assumptions already held or full plan development and implementation.
