Our Services Include: Advanced Directives RN Consultations Healthcare Assessments Individualized Care Plans Infection Control Precautions Nutritional Therapy Wellness Assessments Physical Therapist, Occupational Therapist, Speech Therapist Home Health Aide Services Guardianships, Conservatorship Capacity Assessments to Evaluate Cognition Funeral Arrangements We utilize our resources in the community and collaborate to provide the information and connections to these services.*Virtual Tele-medicine provided over the phone, via Zoom, Skype or Face Time *Assessment reviews infection control, fall and safety precautions, hand washing, healthy distancing, and any signs and symptoms that need to be reported to a medical doctor.*RN reviews all anatomical systems, vital signs, current weight, hydration, nutrition, hygiene, exercise, and activities.* Patient’s mood and cognition is also evaluated.* Skin checks are done that identify skin breakdown, rashes, and areas of concern.* Strict privacy, HIPPA policies are followed.
