Primary Care Home

We were founded in 2017 by Nurse Practioners in Ontario. We use a primary care model that is client centered, and provide comprehensive, private medical care to those who are homebound. We also service those who find that having house-calls based medical care a convenience that allows individuals and families to address their health needs on a busy schedule. Using a multidisciplinary team, our goal is to provide an in-office experience directly to our clients home. We have also branched out in providing house calls to visitors, non-residents and foreign diplomats via 3rd party medical assistance programs and multinational insurance companies. Our company is currently exploring potential in assisting pharmaceutical companies in performing phlebotomy to collect samples on patients involved in drug monitoring.
