Rare Care World

For English scroll down) Stichting Rare Care World is een open informatiebron voor mensen met een zeldzame ziekte of aandoening, hun families en hun zorgverleners. In 2023 zet Stichting Rare Care World zich samen met Stichting Shwachman Support Holland in voor het toepasbaar maken van ICF-vragenlijsten. De ICF* is een waardevol handvat voor cliënten/ patiënten met een chronische aandoening om aan te geven welke activiteiten moeite kosten en welke niet. Individuele scores kunnen opgeslagen worden in een PGO (Persoonlijk Gezondheidsdossier) en gedeeld met zorgverleners. De effecten van nieuwe therapieën,(hulp) middelen en maatschappelijke ondersteuning kunnen gescoord en vervolgd worden.*De ICF (International Classification of Functioning, Disability and Health) is ontwikkeld door de WHO- Stichting Rare Care World is open information source for individuals with a rare disease or disorder, their families and their care providers. In 2023, the Rare Care World Foundation, together with the Shwachman Support Holland Foundation, has committed itself to making ICF questionnaires applicable. The ICF is a valuable tool for clients/ patients with a chronic condition to indicate which activities require effort and which do not. Individual scores can be stored in a PHR (Personal Health Record, PGO in Dutch) and shared with healthcare providers. The effects of new therapies,(aid) resources and social support can be scored and prosecuted.*The ICF (International Classification of Functioning, Disability and Health) was developed by the WHO.
