Reach For Home

Our mission is ending homelessness in Northern Sonoma County. By engaging with community members who are struggling with or at risk of losing housing, we help transform lives with action plans for their success. Our programs promote self-sufficiency, independence, and permanent housing. Through hope, heart, tenacity, and synergy, we strive for dignity for every member of our community. Our services include three program areas: Street Medicine and Outreach, Supportive Housing, and Eviction Diversion. Since 2014, we have provided over 1,200 unduplicated people with services including: housing, winter shelter, wellness checks, utility assistance, rental assistance, or basic necessities like tents, tarps, and hot meals.
