Scodef Care stands to complete the integrated loop between the client, hospitals, GPs, Community Pharmacies and community social care. Scodef care is owned and run by a pharmacist with a wealth of experience and knowledge in the health care sector. At Scodef care, we use a patient-centred pathway to go the extra mile by reaching out beyond the client's personal care to help them manage their medicine more efficiently; by providing adherence-support, consolidating the information, knowledge and explanation they received from their local pharmacy regarding their medications especially when there are changes made to their medicines. This will ultimately lead to:*a strong, holistic support system being made available to the patient to get the most from their medication and be in the best state of health as much as possible.* less hospital re-admission due to non-adherence and medicine errors, minimized emergency hospital visit and improved positive outcome for the client.
