SDP Life works with adults with intellectual and developmental disabilities who are enrolled in the Self-Determination Program. Ongoing independent facilitation services may include, person-centered planning, budget negotiations, spending plan development with spending tracking; assistance in obtaining generic resources, research and implementation of services and supports to help you reach the goals you set for the life you want. Consulting services are available for individuals and professionals requiring added support strategies for navigating healthcare systems, facilitating discussions with multidisciplinary healthcare teams, approaches for obtaining medical related supports, and employment benefits such as Ticket to Work.
