In collaboration with patients, family members, and caregivers, we coordinate patient care between physicians, pharmacists, home care specialists and therapists. Your dedicated care manager will oversee total costs, reduce time commitments and stress for the family, and eliminate oversights in care by clarifying treatment plans, medication regimens and compliance, insurance coverage and other urgent patient needs. Whether you live far away, are unable to take time off of work or simply seek the reassurance of additional support, SMS is available to assist you.
