Side By Side Life Transitions

Side by Side: Life Transitions Coaching for the Later Years offers emotional and spiritual support for individuals who are coping with the challenges of a loved one's aging process and increasing physical or cognitive frailty. We are a resource for those experiencing frustration, confusion and feelings of "being overwhelmed" that often accompany what is known as "caregiver stress". Our support includes offering guidance and assistance with decisions regarding-creating advanced medical directives (living wills)-deciding on appropriate living situations for the older adult -choosing palliative or hospice care -coping with the dynamics of challenging family circumstances Our consultation and coaching provides a setting of empathy and understanding to caregivers and other family members who involved in maintaining their loved one's well-being. In addition we facilitate client and caregiver support groups, offer presentation and workshops on the emotional and spiritual effects of the aging process on older adults and their families, and work collaboratively with other health professionals to enhance and promote comprehensive care of older adults and their families.
