Smith Integrated Care Services provides licensed and insured private care management services in the Boston area. We also provide community education services regarding issues of aging. Our care management services include - Comprehensive assessment and provision and implementation of personalized plans of care. Navigation of health care, including access to diagnostic serviced, coordination with providers for ongoing care, assistance with appointment attendance and follow up, and ensuring medications are consistently reviewed and taken appropriately. Assistance with medical transitions, including finding appropriate settings as client recovers or progresses, and ensuring needed care is available and adjusts as requirements change. Providing information, education, guidance, and support around legal assistance, financial planning, and finding appropriate housing and supports. Delivering education, advice and support to those managing dementia symptoms and their caregivers and families. Encouragement and support to participate in social activities (finding social opportunities and accompanying seniors as requested). Performance of home safety evaluations and education about fall prevention. Assistance with all aspects of home organization, moving, down-sizing and finding and settling in to an appropriate living situation, including logistics around home modifications. Evaluation of eligibility and assistance with application processes for government-subsidized programs or supports (transportation, nutrition, health care, etc.) and facilitation of access to them. Monitoring and advocacy of the care of residents in long-term care settings. Facilitation of difficult conversations among elder and loved ones, including discussions of chronic and terminal diagnoses, changing care needs, advance directive planning, and funeral planning.
