SNF Wound Care providers come weekly to your nursing facility and assess wounds, perform bedside surgical procedures such as bedside debridement's, superficial abscess drainage, G-tube/Supra-pubic replacement and rash or dermarologic evaluation. We help care for all non-healing wounds in the skilled nursing facility setting. Our vision is to partner with your facility to provide exceptional wound care to patients residing at your nursing facility while we strive to achieve fewer appointments to outside clinics, reduced hospital readmission rates and prevent unnecessary ER transfers, for example by implementing our 24 hour G-Tube replacement policy which help reduce costs to your facility. Our partnered facilities receive not only excellent patient care along with 24 hour G-tube replacement but also gain access to all our educational services to make sure your staff is always up to date on the highest standards of care. We understand the concerns and challenges that nursing home administration faces and our wound care providers go through extensive training understanding the fundamental nursing home daily challenges and help coordinate and resolve these issues. With SNF Wound Care providers expertise, we will help avoid Federal and State Survey fines and are present during survey.
